Questions we answer
What you've always wanted to know about your health plan.
These are the four questions employers actually ask us. Each one is answerable from data you already have or already own — and each has a number attached.
- Am I paying market rates — or is my competitor paying less?
The fees nobody itemized, sitting in your own filings — broker commissions, admin loads, spread you never agreed to.
$2,700 / employee
One employer's annual broker fees alone — before anything else.
- Were we actually billed correctly?
Out-of-network claims paid as in-network. Duplicate and above-contract charges. And the deeper one: did the care I paid for in claims actually get delivered?
4% → 12.3%
Postpartum-hemorrhage diagnoses jumped while transfusion rates stayed flat. Care billed, not delivered.
BHI / BCBSA, 2026
- The vendors promising to save me money — are they actually doing it?
Is anyone even using the point solution you pay for? Are they closing the gaps they were hired to close, or grading their own homework?
128 members
Pre-diabetic and not enrolled in the diabetes program you already pay for.
- Is the care any good — and did my people actually get better?
Are there better providers I should be steering my team to? The deepest check nobody runs.
2× the benchmark
The surgical group your employees use most has a repeat-procedure rate double the regional norm.
40% above peers
MSK spend runs 40% above peers — with no PT-first pathway. Build the plan around who your people actually are.
22 points below
Cancer-screening completion below benchmark — the rare fix that's the right thing and the cheap thing.
About the scorecard
What people ask before they run one.
- Are you a broker, or will my broker find out?
- No. We don't sell insurance, administer plans, or manage drug benefits, and we take no commissions or fees from anyone who does — which is the only reason an independent read is possible at all. Your scorecard goes to you and nobody else.
- What do I have to give you?
- An email address. Every number comes from public government records and published surveys. We never see anyone's medical records, and there's nothing to sign.
- Is the free version the real version?
- Yes. The scorecard is complete on its own. Reading your contract, checking individual bills and recovering money are separate paid work. No spam. No obligation.
- Who is behind this?
- Grounded Health Inc. — a public benefit corporation. Our methodology is published and versioned, so you can always double-check our work.

How to read our numbers.
- 1. Scorecards are built only from public government filings and published price data. We never receive, store or process anyone's medical information.
- 2. Every dollar figure we publish is a conservative floor, not a ceiling, and the range reflects how closely we can match comparable companies. On a scorecard, a card headed “Example scorecard” is a real employer drawn from its own public filing; one headed “Sample scorecard” is invented.
- 3. Health benefit costs are projected to rise 9.0% for employer plans in 2027. Source: PwC Health Research Institute, “Medical cost trend: Behind the numbers 2027”, June 2026 — a survey of 27 health plans covering more than 103 million people with employer coverage and 8 million on ACA marketplaces. Projected 2027 increase: 9.0% for employer plans, 8.5% for individual; the 2026 projections were restated upward from 8.5% and 7.5%.
